This form is for those just starting with me. This is the documentation required for a full historical intake. Typically this is filled out and brought to our first session by the client.

The liability waiver and statement of intent is mainly to communicate how I work and what my intent is around our consultations. There are several liability parts at the end of the document. Typically, I provide these at our intitial consult and review with the client. It is then signed at that time.
